Adult Day Services are group programs that, through individual care plans, help to meet the needs of adults who have physical or cognitive impairments. Participants attend an on-site program one or more days per week on a regular basis during specified hours. They then return to their own homes for the remainder of the day and night. Meals, structured group programs, as well as opportunities for individual activities, are offered. |

| Coordination of Care - Care Managers and Consultants

Care managers/consultants help people navigate available service options.
They provide an assessment of the persons emotional, social, functional, financial and physical status and make recommendations on available services and interventions. This may include monitoring ongoing services or assistance in seeking alternative housing arrangements or Nursing Home placement. |

Also known as Skilled Nursing Facilities (SNF), Nursing Homes provide 24-hour medical and social services to the frail, elderly and chronically disabled. These residential facilities represent the most complex level of senior care and provide services to those persons who need highly skilled medical care and supervision. Many Nursing Homes have Transitional Care units that serve those in need of short term rehabilitation. Many also have Alzheimer's/Special Care Units designed to serve individuals with memory impairments. |
