Evaluation - PRI and Screen/Nursing Assessment    click to view

PRI/Screen (Patient Review Instrument/Screen) is an assessment required in New York State to determine eligibility and appropriate level of care for those seeking skilled nursing home placement. It must be completed as part of the facility's application process by a specially trained registered nurse. (The Screen component may be completed by a specially trained social worker.) For patients in the hospital, the PRI/Screen will be completed by hospital staff. In the community, any of the agencies listed may provide the service.

Terminal Care - Comfort Care/Palliative Care    click to view

Terminal Care is a term used to describe both Comfort Care and Hospice Care as types of care which seek to provide physical care and counseling to terminally ill patients and their families with the goal of symptom management rather than cure. The terminally ill individual has made a decision that he or she does not want medical intervention or hospitalization, but chooses to be made comfortable for the remainder of their life through the use of pain management and other measures. Care is provided by nurses, physicians, home health aides, medical social workers, spiritual care, and volunteers. There are two types of terminal care, Hospice Care and Comfort/Palliative Care. The two types of care differ in payment mechanism, prognosis for life expectancy, and where the care can be provided. Comfort/Palliative Care is a philosophy which embodies the goal to provide comfort to the terminally ill, rather than medical intervention or cure. It differers from Hospice care in that it is paid for privately, through long term care insurance, or by Medicaid. Some nursing homes have special units, beds, and staff-t0-patient ratios to provide this care.

Hospice Care is a Federal program paid for by Medicare (or health insurance benefits if they person is under 65) and as such, the services must be provided by a Certified Agency which manages and provides approval for all aspects of care. Care may be provided in a person's home or within a nursing home setting. The patient's prognosis can be as long as 6 months. As a capitated program, the individual relinquishes their Medicare benefits to the agency providing services. The program pays for the medical, nursing, and physical care only, not room and board.
